Eden Country Care’s Care Coordinator nominated in national awards
This prestigious nomination highlights Sam’s exceptional dedication to her clients and her ability to provide compassionate, hands-on support for those with complex care needs.
A particularly remarkable example of Sam’s work is the care she coordinated for a client who lived with Fibrodysplasia Myositis Ossificans Progressiva (FOP), a rare and severely disabling condition that progressively turns connective tissue into bone. The client lived in a remote village and was supported by Eden Country Care for over 22 years. He sadly passed away in June 2024 at 52 years old, far surpassing the average life expectancy for individuals with FOP, which is typically around 40 years.
As the client’s condition advanced, Sam played a crucial role in coordinating complex care, working closely with district nurses, GPs, occupational therapists, and the Newcastle Ventilation team to ensure that every aspect of his care was meticulously tailored. Sam’s approach went beyond simply managing his medical needs, she helped to enhance his quality of life by promoting his independence, autonomy, and personal interests. For instance, despite his limited mobility, Sam and her team supported his annual trips to Blackpool, enabling him to enjoy the seaside and socialise beyond his home.
The client's preference to remain at home, even during severe illnesses, was always respected by Sam and her team. Their unwavering dedication ensured that he could continue living in his home, surrounded by his family and familiar surroundings, rather than being transferred to a hospital. As the client’s condition worsened, Sam coordinated additional health training for carers, including specialised training for the NIPPY ventilator, which supported his respiratory function during critical moments.
After the client passed away at home, Sam and the Eden Country Care team continued to offer support to his family. The team was asked by the family to provide dignified post-death care, a task that Sam and her colleagues undertook with great compassion and sensitivity. At the client’s funeral, one of his primary carers was honoured to act as a pallbearer, a testament to the deep connections formed over years of care.
Reflecting on her role, Sam shared:
"For me, home care is about more than just providing physical support, it’s about really getting to know clients, understanding their wishes, and making sure they feel safe, respected, and cared for in their own home. I feel privileged to be part of their lives and to help them maintain their dignity and independence."
